Welcome to iPrep Academy! We are excited to have your child join us for their educational journey.  Below you will find documents you will need to complete, sign and return to the school so that we may process your child's registration. Registration for Students NEW to Miami-Dade County Public Schools, Private School Transfers, Pre K, and Out of County Transfers: 
  • Age and Legal Name Verification
    •  Original Birth Certificate
    • Duly attested original Birth Certificate or Birth Card - must be original; hospital certificate not acceptable. 
    • Passport or Certificate of Arrival in the U.S. showing age of child (FM-6670 cannot be photocopied), may also  be accepted. 
  • Verification of Address
    • Utility Bill(power, water or gas) showing parents name and service address including zip code.
    • Broker's or Attorney's statement of parent's purchase  of residence or properly executed lease agreement. 
  • Copy of School Records (Report Cards, Test Scores etc.)
  • Health Requirements (DH3040 & DH 680)  - from private doctor or local health provider. 
  • Completed Magnet Contract
  • Completed & Signed  Directory Information  Opt-Out Form (FM-6479)
  • Completed and signed Disclosure at time of registration (FM-5740)
  • Completed and signed Home Language Survey Form (FM-5196)
  • Completed and Signed Registration Information Form
  • Original Eligibility and Enrollment Certificate (Pre-K Only)
  • Parent or Legal Guardian must bring a withdrawal slip from sending school
  • Proof of Address in name of Parent/Guardian

All students:
  • Magnet Agreement
  • Declaration of Magnet Acceptance
  • School Records & Transcripts 
  • FLVS transcripts (9th & 10th Grade) - if applicable
